TIQUE PCP Project: the technological innovation revolutionising care for heart failure patients

Objective of TIQUE PCP

Izertis is leading the implementation phase of Plat4TIQUE in three European countries (Spain, Italy and Sweden), through an innovative and comprehensive care solution that provides rapid responses to the needs of complex patients with advanced heart failure. The initiative focuses on real-time monitoring of patients' condition, prediction and prevention of frequent decompensation, reduction of hospital admissions, and optimisation and alignment of the care process by improving patient transitions between care levels. Plat4TIQUE, led by Izertis and comprising Additum, Vitalera and Sferanet, encourages the participation of patients and their caregivers, educating them to improve adherence to treatment and self-care. 

TIQUE PCP to apply AI-based technologies to help prevent heart failure

Izertis, as the technological leader of the project and an expert in emerging technologies, will apply artificial intelligence-based technologies to prevent frequent decompensations in heart failure patients. Through a decision support system, clinicians will be able to personalise patient care, increasing process efficiency and quality of care. This project not only represents a qualitative leap in healthcare, but also contributes to the transformation of European healthcare systems towards a more efficient, integrated and patient-centred model.

TIQUE (InnovaTIve care services to deliver QUick rEsponses to complex patients with advanced heart failure through integrated care) is a Pre-Commercial Public Procurement (PCP) project funded by the European research and development programme Horizon 2020, under contract GA 965356, with the participation of European public-private entities. It aims to respond to the challenges of 3 healthcare organisations that define the needs in the management of patients with heart failure and validate solutions: the Hospital de Sant Pau (Spain), l'Azienda Sanitaria Locate Avellino (Italy) and Region Västerbotten (Sweden). The project also involves the participation of the Catalan public health system through the Fundació TIC Salut and the Agència de Qualitat i Avaluació Sanitàries de Catalunya (AQuAS). The importance of TIQUE PCP is that heart failure affects 62 million people worldwide and is the leading cause of hospitalisation in people over the age of 65, placing a significant burden on health systems. The lack of adequate and coordinated management between the different levels of care, as well as poor patient monitoring, increases the risk and frequency of acute episodes, affecting the patient's quality of life and worsening their clinical condition.

Project development and implementation of BeatBeat

During the previous phases of the TIQUE PCP project, our consortium has developed the BeatBeat by Plat4TIQUE solution, a platform that represents a pioneering approach to address the complex challenges of managing and caring for patients with Advanced Heart Failure. BeatBeat is a platform that facilitates an integrated care model, designed to optimise the care process for different types of patients and their transition between different levels of care. It also facilitates the integration of multidisciplinary teams to provide holistic care, minimise waiting times, and ensure effective communication through telemonitoring and telemedicine. 

The BeatBeat solution consists of several components: 

  • A patient-centric mobile application for patient monitoring, activation and telecare.
  • Aclinical data collection and visualisation tool for healthcare professionals to monitor, manage, and personalise care. 
  • Clinical dashboards for real-time visualisation of the most relevant indicators for the correct monitoring of patients and support in clinical decision-making.

These 3 components of the BeatBeatsolution, consolidate an ecosystem (Virtual Care Centre) managed by the different healthcare professionals, allowing to optimise care, increase efficiency and improve patient outcomes, promoting a culture of value-based healthcare integration (VBHC) within healthcare institutions.

On a technological level, the end-to-end solution provided by BeatBeat will facilitate the exchange of information between medical staff at different clinical levels, including doctors, nurses and other healthcare professionals, promoting behaviours that improve patients' quality of life. Thanks to this platform, personalised lifestyles will be proposed for each patient, taking into account their specific individual needs.

The platform's flexibility allows the integration of different tools, applications and wearables, fostering communication, support, training and monitoring thanks to its hybrid service model. This application has been designed using advanced technologies that promote the use of digital identity, cybersecurity environments and is aligned with the European Health Data Space and GDPR.
