Izertis leads the PLAT4TIQUE consortium in the European TIQUE PCP project to improve the quality of life of patients with heart failure
TIQUE PCP, the pre-commercial procurement in which the PLAT4TIQUE consortium, led by Izertis and comprised of Additum, HumanITcare, and Sferanet, is involved, is developing an innovative solution to improve the quality of life for patients with advanced heart failure. This comprehensive approach is based on the implementation of emerging technologies, optimization of the healthcare process, and continuous care, with the goal of personalizing patient care, enhancing their experience and autonomy, and facilitating coordination among healthcare professionals.

Currently, the PLAT4TIQUE consortium, composed of Izertis, Additum, HumanITcare, and Sferanet, is immersed in the execution of the second phase of this PCP. But what exactly is TIQUE PCP and why is it so crucial? TIQUE PCP is an innovative and integrated healthcare service that provides rapid responses to complex patients with advanced heart failure. This project is funded by the European research and development program Horizon 2020, under contract GA 965356, and involves the participation of European public-private entities.
The initiative focuses on real-time monitoring of patients' conditions, prediction and prevention through technology of frequent decompensations in these patients, and the reduction of hospital admissions. Additionally, the PLAT4TIQUE solution encourages and promotes the active participation of patients and caregivers, educating them for better adherence to treatment and personal care. The relevance of TIQUE PCP lies in the fact that heart failure is the leading cause of hospitalization in patients over 65, imposing a significant burden on healthcare systems. The lack of proper and coordinated management between healthcare levels, as well as the lack of accurate patient monitoring, increases the risk and frequency of acute episodes, impacting the patient's quality of life and worsening their clinical condition.

Additum contributes its experience in creating value in the healthcare sector, seeking the optimization of healthcare processes and coordinating the work of different clinical units and healthcare professionals. HumanITcare provides its API and knowledge in technology applied to remote personalized health monitoring, while Sferanet offers expertise in AI and health projects as an innovation partner.
